Feature Overview

What is the expected behavior of this feature?

  • The AQ Red Flags workflow is a clinical safety mechanism designed to identify and manage members who present with medium-risk clinical indicators during the onboarding process. This workflow ensures appropriate clinical oversight and intervention for members who may require additional attention before starting their exercise therapy program. The AQ Red Flag workflow in Care Hub is specifically designed for: Chronic/CORE, UNIFIED, FALL PREVENTION (EMO) and are triggered when certain flags are identified in their application questionnaire (AQ). Here are the key components of the workflow:

    1. Overview of AQ Flags:

      • The updated AQ Flag PT Workflow applies to chronic members and unified member, including those with Fall Prevention/ chronic balance.
      • ACUTE AND WPH will NOT see AQ Red Flags at this time.
      • Members are auto-accepted into the program if they are considered low risk (see list below). They will be auto sent a message when auto accepted with low priority red flags.
    2. Workflow Steps:

      • Access the PT Dashboard in Care Hub and click on the “AQ Flags” tab under the Clinical Workflows section.
      • Review all AQ Flags, particularly those related to exercise limitations.
      • Outreach to associated members via outbound messaging from PT or MM in PT thread and workflow will auto complete the workflow.
    3. Member Interaction:

      • If a member reports concerning symptoms, the PT should use clinical judgment to determine the next steps, which may include reaching out to the member to gather more information or to confirm their safety for participation.
      • Responses to members should vary based on the level of concern, with specific saved replies for different scenarios (e.g., no concern, need for follow-up, or requiring clearance).
    4. Handling AQ Red Flags:

      • If a member indicates red flags that impact their safety, the PT must notify the coach to pause outreach and follow up appropriately.
      • The workflow emphasizes the importance of clinical judgment in navigating member interactions, especially when ambiguity arises.
    5. Workload Management:

      • The workload for AQ Flags can fluctuate, and PTs should communicate with their managers if the volume of reviews becomes unusually high.
    6. Mass Messaging Considerations:

      • Open workflows, including AQ Red Flags, generally block mass messaging to ensure personalized communication with members.
    7.  Workflow Triggers for the AQ Red Flag:
      • Application type must be Chronic/CORE, UNIFIED, FALL PREVENTION (EMO).
      • Member must have an active bucket with 2 low risk flags NOT 3.
        • Exercise Restrictions (exerciseRestrictions)

        • Other Conditions (otherConditions)

        • Bowel/Bladder Changes (bowelBladderChanges)

        • Groin Numbness (groinNumbness)

        • Limb Numbness or Weakness (limbNumbnessOrWeakness)

        • Fevers/Chills/Night Sweats (feversChillsNightSweats)

        • Redness/Warmth/Swelling (rednessWarmthSwelling)

        • Unexplained Weight Loss (unexplainedWeightLoss)

      • 3 High Risk Flags require immediate clinical attention and will NOT trigger the AQ Red Flag.
        • Falls from Limitation Without Clearance (fallsFromLimitationWithoutClearance)

        • Trauma Without Clearance (traumaWithoutClearance)

        • Recent Surgery Without Clearance (recentSurgeryWithoutClearance)

        • Upcoming Surgery (upcomingSurgery)

        • Wheelchair Mobility (wheelchairMobility)

        • Sit to Stand Assist (sitToStandAssist)

        • High Risk Pregnancy (highRiskPregnancy)

  • This workflow is crucial for ensuring that members are monitored effectively and that any potential risks are addressed promptly, allowing for safe participation in the Hinge Health program

  • FAQ:

    • New members who have AQ red flags but the automated intro message gets sent out before they can respond to the AQ red flags.

      • Looking at Care Hub, the AQ Workflow may have been closed on (Sunday) and the member got pulled into Monday reporting for a message.

      • If the workflow is cleared but a message is scheduled, they may still get pulled into MM before the scheduled messages send.

      • It would help to send the message in real time OR leave the AQ workflow open and have the scheduled message send and then auto clear the workflow.

Troubleshooting

What general best practices can I implement before filing a ticket?

  • Check to see if the member is a Chronic, WPH OR Acute member.
  • IF this is an Acute member, and there is no red flag, this is working as designed.
  • IF this is a Chronic member with red flags and no red flag workflow triggers after reading the details above, then follow the guidance in Ticketing Best Practices to file a ticket in Care Hub.
